Prevalence and Usefulness of D-dimer of Deep Vein Thrombosis in Brain Injured Patients

Abstract
Background: To investigate the prevalence and risk factors of deep vein thrombosis (DVT) in brain injured patients and evaluate the usefulness of D-dimer test.

Methods: We investigated two hundred sixty nine patients with brain injury. Age, causes of brain injury, gender, fracture of lower extremity in traumatic brain injury, a pattern of paralysis, muscle power, heterotopic ossification, pressure ulcer, diabetes mellitus and heart disease as possible risk factors were investigated. Duplex ultrasound was done for patients who were suspicious to have DVT. Sensitivity, specificity, positive and negative predictive value of d-dimer were analyzed.

Results: Seven out of two hundred sixty nine patients (2.60%) were diagnosed as DVT. In patients with DVT, weaker muscle power, more proportion of diabetes, heart disease or hemorrhagic stroke was found. The sensitivity of d-dimer was 71.4%, the specificity was 55.6%, the positive predictive value was 92.5%, and the negative predictive value was
20.0%. In addition, semiquantitative results of d-dimer test had a trend of positive correlation with the prevalence of DVT.

Conclusion: We considered d-dimer test as a useful screening tool and thought that further study was needed for diagnostic value related to semiquantitative results of d-dimer.
